The information is recorded in the sales service and accessed by the responsible team. You can review the summary before sending and download a copy afterwards.

Med is a product of Catalisa Tecnologia e Dados LTDA. To request access, correction or deletion of your data, use the privacy contact listed in Catalisa’s policy.

We use Cloudflare Turnstile to verify submissions and reduce automated contacts. Verification processes technical browser and connection data.

Use the form to describe your operations and priorities. Patient information and clinical documents belong in your clinic’s care channels.

Images and interfaces illustrate work situations. People, clinics and times in these compositions are examples.
Navigation analytics.
With your permission, we use Google Analytics to understand page visits, plan choices and form steps. Contact details, clinic information and typed text stay out of analytics. You can change your choice in privacy preferences.
Language preference.
A technical site-version identifier helps your browser refresh cached files.
Your language choice is saved in this browser for your next visit. Form fields are not saved with this preference. We also use a temporary session to protect form submission.
